“Everyone in our group just immediately started saying, like, ‘Oh, I’ve been through that too,’ ” Swift said. “And I thought to myself, if this has happened to all of us, it’s such an interesting angle on humanity and relationships, and I wonder if I could write something from the perspective of getting this phone call, and really only from the perspective and the side of the phone call of the ex who’s been through it before.”

The gothic story line in “Patient Zero” appears to draw inspiration from Daphne du Maurier’s 1938 novel Rebecca.

The book follows a young woman who marries wealthy widower Maxim de Winter and moves into his sprawling English estate, Manderley, where the memory of his late first wife, Rebecca, looms over their relationship. As the story unfolds, questions surrounding Rebecca’s death — and the man both women loved — come to the forefront.

Swift previously mentioned Rebecca while discussing the types of stories she loves during a December 2025 appearance on The Late Show with Stephen Colbert. She described being drawn to stories involving an old British mansion, a mysterious relationship, a man who “may not be what he seems,” a past murder and the possibility of a ghost.

Those elements bear a striking resemblance to “Patient Zero,” which stars Johnson as the new woman in Farrell’s life while Swift plays his late former partner, whose ghost attempts to warn Johnson about the danger she faces.

She teased several celebrity cameos

Swift recruited several famous faces for the “Patient Zero” music video — and it turns out she teased their appearances weeks earlier.

During a skit at the 2026 Emmys, Swift looked directly at the camera and mysteriously said, “Saccharide. Aries. From the vineyard. North or south.”

After the music video premiered, fans cracked the code: “Saccharide,” or sugar, pointed to Farrell, who stars in Sugar, while “Aries,” represented by a ram, hinted at Los Angeles Rams quarterback Matthew Stafford’s surprise appearance.

“From the vineyard” nodded to Cara Delevingne — “de” means “from” and “vigne” means “vine” in French — while “north or south” pointed to Johnson, a play on North and South Dakota.
